Brief Description This module covers the underpinning knowledge and skills required to enable practitioners to provide effective care for people living with a long term medical condition within a variety of care settings. Students will be encouraged to consider the holistic implications of living with a long term condition and explore the strategies involved in the case management requirements of this group of people. They will also develop a deeper understanding about selected long term conditions. This module is mapped to the National Occupational Standards.
Indicative Syllabus

This module will cover the following topics:

  • the psychological affects of living with a long term condition;
  • the sociological effects of living with a long term condition;
  • aetiology and disease progression for a variety of long term conditions, for example Heart Disease, Coronary Vascular Disease, Parkinson’s Disease, Respiratory Disease, Multiple Sclerosis, COPD and Diabetes;
  • the National Service Framework for those with long term conditions;
  • underpinning themes running through all sessions will include accountability, communication skills, record keeping, the promotion of dignity and health, working in partnership with others including carers and those from other sectors.
Learning Outcomes

i. Knowledge and Understanding
On successful completion of the module students should be able to:
a) Outline the aetiology and disease progression of selected long term progressive medical conditions;
b) Identify the psychological implications of living with a long term condition;
c) Assess the implications of the stage of progression of progressive long term conditions for the care needs of an individual;
e) Analyse the ‘good practice’ (NSF) legal and ethical issues involved in the care of people with long term conditions.



ii. Skills
On successful completion of the module the student should be able to:
a) Work in a team in the workplace or at the University;
b) Plan an activity;
c) Analyse, synthesise and then present information orally to others;
d) Communicate in writing.

Staff/student contact:  20% (lectures, workshops and tutorials)
Student directed learning: 60%
Work based learning: 20%
