Brief Description

This module provides opportunities for students to develop knowledge, critical insight and skills in the management of long-term conditions. It adopts a client centred approach, and evaluates the pathophysiology of long term conditions/disease pathways. The module will concentrate on those areas of illness/disease highlighted by the current health agenda.

Indicative Syllabus

This module will cover the following topics:

  • the client journey through the disease process;
  • the application of holistic health and social care assessment;
  • the physiological and psychological aspects of acute episodes upon clients in social and clinical settings;
  • the psychological impacts on carers and families in managing the acute episodes of their clients/family members;
  • management of long term  illness, including respiratory, neurological, metabolic, psychological and circulatory conditions;
  • multi-professional and multi-disciplinary working in support of the person with a long term condition;
  • the evidence base used to reform and enhance practice.
Learning Outcomes

A student passing this module should be able to:

  1. Critically explore the pathophysiology of a long term condition and its clinical complexity.
  2. Critically appraise the effects of long term conditions on the client, family and carer in their social setting;
  3. Critically analyse and provide examples of managing specific disease/s appropriate to their area of practice including the challenges of multi-agency intervention.
  4. Critically evaluate long term disease progression and the importance of case management.
