Brief Description

This module explores the underpinning knowledge and skills required to provide effective care for people living with a long term condition within a variety of care settings. Students will be encouraged to consider the holistic implications of living with a long term condition and explore the strategies involved in caring for an individual who has a long-term condition or co-morbidities.

Indicative Syllabus

This module will cover the following topics:

  • The aetiology and epidemiology of a variety of long term conditions, for example Dementia, Heart Disease, Coronary Vascular Disease, Parkinson's Disease, Respiratory Disease, Multiple Sclerosis, COPD, and Diabetes;
  • the psycho-social , physical , emotional, mental capacity and wider environmental effects of living with a long term condition;
  • the political and professional influences on care management;
  • the importance of accountability, communication skills, record keeping, the promotion of dignity and health, working in partnership with others including carers
  • the use of assistive technologies and their role in assessment  and management of care.
Learning Outcomes

A student passing this module should be able to:

  1. Outline the aetiology and disease progression of selected long term progressive conditions;
  2. Identify the psychological and social implications of living with a long term condition, including those related to mental capacity;
  3. Assess the implications of the stage of progression of a long term condition for the care needs of an individual;
  4. Analyse the elements required for high quality care, demonstrating knowledge and understanding of professional guidelines and political directives; 5. Understand the contribution of assistive technology to living with a long term condition.
